Hello Readers,
I have posted a copy of my dissertation survey below. You should be able to easily scroll through the document to preview it. Alternatively, you may download a copy in ".doc" or ".pdf" format. I strongly recommend that you download the survey in ".doc" format, which will make it easier to fill in your responses.
To download a copy of the survey:
1) Simply click here.
2) Click on the green "Download" button on the black bar at the bottom of the page.
3) Select the document format you prefer and click "Download Now".

Instructions for completing and submitting the survey are included within the document. In order to complete my dissertation on time, it is imperative that I receive all survey responses by 30 June 2010. If you have any questions, please feel free to email me: seandiamondsustainability@yahoo.com.

In order to thank survey participants, I intend to offer all participants a free digital copy of my final dissertation write-up, which will likely include aggregated, unattributable data, prior to any potential publication or public disclosure. While the advance copy of my dissertation will be solely for internal use, it is my hope that survey participants will find this data useful in making decisions in the near future.
